Hall of Fame Nomination Form

Forms must be submitted by April 11, 2008 to be considered for the Induction Ceremony. Packets of those nominees not chosen will remain under review by the Selection Committee for two years. You may submit your Hall of Fame nomination by filling in the web form below and clicking 'Submit Nomination'. Or, by sending a completed nomination packet to:

Letters of Recommendation: Maximum of three, one page each, are recommended to be included with the nomination packet. Nominee may not nominate himself or herself. Again, nominations will be accepted from current hall of fame members, alumni, and staff at Menlo College.
